→ Fires whenever system asks for approval or file access

- Separates permission prompts from ordinary notifications and completed turns from successful tasks.
- Maps the two events to short, distinguishable character voice clips from available packs or audio you supply.
- Prevents duplicate playback from overlapping hooks, repeated events, and multiple integrations.
- Diagnoses silent alerts across local terminals, SSH, containers, and machines without an audio session.
- Verifies actual event-triggered playback and preserves existing hooks when applying changes.
